Thank you for filling out this questionnaire. It has been designed to help us decide if you and your family are adequately prepared to discuss the serious responsibility required when adopting a dog. Our primary intention is to place each dog in the best possible home based on his/her needs. We are asking you to make a lifetime commitment to the dog you adopt. It is very stressful for a dog to be returned. Because of this, we will ask that you do not adopt unless you are absolutely certain you and your family are ready.  Must be 21 or older to adopt, and home check (physical or virtual) required. Please note submitting this application does not guarantee approval to adopt.
